Targeted Case Management

TCM is a specialized service designed to help children and youth with significant behavioral health, emotional, or developmental needs gain access to the services and supports they require. The goal of TCM is to ensure that children and families receive coordinated, individualized care, especially when services span multiple systems (e.g., mental health, education, child welfare, juvenile justice).

Adult Case Management (CI)

Adult Case Management is a service that helps adults with serious mental illness access, coordinate, and maintain the services and supports they need to live safely and successfully in the community. The primary goal is to promote recovery, stability, and independence by ensuring individuals receive comprehensive, coordinated care across multiple service systems.

BHH Services

BHH Case Management refers to services provided through a Behavioral Health Home (BHH) program. A Behavioral Health Home coordinates mental health care, physical health care, and social services for people with serious mental health conditions. BHH case management = integrated care coordination that helps people with serious behavioral health conditions manage their health, services, and life needs in one coordinated system.

Outpatient Counseling

Outpatient Counseling Services are structured mental health services provided to individuals, families, or groups who need therapeutic support but do not require 24-hour supervision or hospitalization. Services are delivered in a clinic, office, school, community setting, or via telehealth. The goal of outpatient counseling is to improve emotional, behavioral, and psychological functioning while helping individuals develop coping skills, manage symptoms, and enhance overall well-being.
